► If entering the APMVC VOCAL SCHOLARSHIP, you will need:

• Vocal Scholarship Letter2013

• Vocal Scholarship Entry Form 2013 - (printable pdf version -print, complete and post the signed copy)

OR
• Vocal Scholarship Entry Form (b) 2013 - (Fill in online pdf version - type in details, print and post the signed copy)

* NOTE - Any data entered on this form online will NOT be saved on your computer - it needs to be printed at the time of filling in *
It is recommended that a duplicate copy is printed at the same time for your own reference.

• VERIFICATION OF NOVICE ELIGIBILITY(b) - (Fill in online pdf version - type in details, print and post the signed copy)

• FORM NO. 1 - Performer’s Declaration Form for Eisteddfods and Music Competitions
(There is also a Form 1(a) specific to the Contemporary Vocal Section)

• FORM NO. 2 - Temporary copy for use by an adjudicator in an Eisteddfod/Music Competition

• FORM NO. 5 - Temporary copy for use by an accompanist in an Eisteddfod/Music Competition
